Please see below for information for Camper Health Forms and Travel Forms for 2012 Camp season.
Health Forms: Our Health History Form is online. Complete the Form online through your CampInTouch account. The Physician’s Authorization Form, Immunization Form and Parent Authorization Form are available on your CampInTouch account and are individually barcoded. When the forms are completed and faxed to Camp, they will be automatically stored in your daughter’s file. The Physician’s Page must be completed and signed by your daughter’s Physician. All Health Forms are due to Camp 6 weeks prior to your daughter’s arrival.
Travel Forms: If your daughter will be traveling without you by air or by train to camp, please complete the Camper Travel Form through your CampInTouch account not later than 4 weeks prior to your daughter’s arrival.

Rules and Policy Form : This form is handed out on Opening Day. If you are not bringing your daughter to camp on Opening Day, please download this form and fax or email to camp prior to your daughter’s arrival.
Parent’s Permission Form: This form must be completed if someone other than the camper’s parents or guardian will be picking their daughter(s) up from camp.
Permission Form to Carry and Self Administer Emergency Medication: This form must be completed for any camper who must carry medication with them throughout the day. This would include medications such as inhalers and epi pens.
Packing Lists
The following packing lists are provided for your convenience. It is intended as a guide. Clothing is laundered once during a two week stay by a commercial laundry. Do not bring clothing that requires special handling. Permanently label each article of clothing, shoes, and personal items with the camper’s name.
The clothing can be packed in a footlocker and the linens in a laundry or duffel bag. The footlocker should not be higher than 14 inches so it can be stored under the bed.
